PATIENT OF THE YEAR
(Guidelines)

Purpose:
  • To select a patient who exemplifies the desired outcomes of cardiopulmonary rehabilitation.
  • To recognize the patient representative at the annual symposium as a positive role model for other patients, staff members and those involved with rehabilitation.
Guidelines:
  • Each program may submit one patient annually at the Board of Directors meeting held at the SC Medical Association headquarters or submit information to a designated member who will compile an information sheet listing the nominees information (outcome data, motivation/attitude, staff's outlook, etc.). The names of the patient and program will be excluded to prevent bias. Patient nomination forms will be issued at the August meeting. Deadline for submission is November 1.
  • The information on the nominees will be issued at the November board meeting. The directors will review the patient information and vote for the "Patient of the Year" via mail or email by December 1.
  • Each nominee will receive a certificate of congratulations from the SCCRA and individual programs are encouraged to celebrate their patient's accomplishment.
  • The patient chosen will receive one night's accommodation at the Charleston Place Hotel and will be recognized at the symposium. Expense money for food and gas will be included.
  • The President of the SCCRA will present a commemorative plaque to the patient during the award ceremony at the annual symposium.
  • The SCCRA Patient of the Year will be submitted to the Ecotrin Heart Patient of the Year program if applicable.

  • August Board Meeting - Nomination form
  • November 1 - Deadline for nomination
  • December 1 - Deadline for vote
  • January 1 - Announcement of selection

SOUTH CAROLINA CARDIAC REHABILITATION ASSOCIATION SERVICE AWARD

Listed below are the award guidelines and the past recipients:
Guidelines
  • SCCRA will present annually to a member of the SCCRA or a person in the state of South Carolina to recognize his/her contribution to the rehabilitation of cardiopulmonary disease.
  • The award will consist of an inscribed plaque to be presented at the annual meeting.
  • During the fall meeting of the Board of Directors nominations will be solicited. The names of nominees will be forwarded to The Executive Committee. The Executive Committee will determine the finalist, and at the annual symposium the President of the SCCRA will announce the winner.
  • The following recommendations are considered for nominations:
    Must be active within the state for five (5) years.
    Must have made some type of significant contribution to the development, implementation and enhancement of cardiopulmonary rehabilitation programming within the state of South Carolina.

SCHOLARSHIP ADMINISTRATION POLICY FOR
FORMULATED: APRIL 2000
Purpose - To provide for scholarship activity sponsored by the South Carolina Cardiopulmonary Rehabilitation Association for the benefit of the Association and its constituency; To promote the professional development of individuals, programs, and the Association; To advance the professional field of cardiovascular and pulmonary rehabilitation within the state of South Carolina

Scope - The South Carolina Cardiopulmonary Rehabilitation Association will provide Scholarship funds which may be applied to any activity of professional advancement deemed acceptable by the Executive Committee of the Association.

Executive Committee Responsibilities - The selection and execution of the award is the authority and responsibility of the Executive Committee. The applications will be received by the designated administrative personnel of the SC Medical Association and forwarded to the Executive Committee Chair. The Executive Committee will serve as the financial executor of the scholarship concerning the following issues:

1. Annual analysis of the Association's financial standing.
2. Submit approval to the Board of Directors for the awarded amount based on the financial analysis.
3. The Executive Committee has discretion to disperse from the approved funding to one or more selected applicants.

Applicant Qualifications -
1. Member in good standing of the SC Cardiopulmonary Rehabilitation Association - no delinquent dues.
2. Serves as a positive role model for the profession and his/her peers.
3. Shows evidence of professional commitment through membership and involvement in local, state and/or national organizations.
4. Presents a qualified proposal which contributes to personal , professional, or organizational advancement in the field of cardiopulmonary rehabilitation.

Scholarship Amount - Will not exceed one thousand dollars. The amount will be determined annually by the Executive Committee based on the financial status of the Association during the year of the scholarship term.

Term of the Scholarship - The term of each award shall be for a period of one year. Recipients of the award are not precluded from making future applications.

Types of Scholarships -
1. Professional Advancement
Examples: Attend state/national conference, obtain professional certification, educational needs, etc.
2. Research
Advancement of knowledge of practice in the professional field.
3. Program
Innovative implementation of program related functions Examples: Funds needed to develop tri-fold forms for documentation, advertising set up fees, publishing/graphics costs, etc.
4. Other
Will be considered and reviewed at the discretion of the Executive Committee.
